I. General Information

All transcript requests must be made by the student themselves, either in person or in writing by letter or email.  Subsequent transcripts will be subject to a minimum fee of 500 pesos, payable in advance by cash.  Requests should be made in a timely fashion (at least two weeks in advance). Requests will only be entertained during office hours, Monday to Friday.  Official transcripts will only be sent to recognized academic institutions and to other institutions to satisfy legal documents. Other legal documents, such as certificates, will be subject to a minimum fee of 100 pesos, payable by cash. Transcript requests are available only from February to May and September to November. It is advised to make the request during the academic year to avoid delays.

II. Instructions

1. Complete the form below, and we will automatically generate your request.
2. Pay the corresponding fee to Word of Life Bible Institute. (Applicant must settle any outstanding balance before applying for a copy of his/her Transcript of Records.)
3. Applicant will incur the mailing cost and other fees, such as postal fees and other delivery fees.

III. Reminders

1. Applicants must claim their documents personally. Should they be unable to do so, they may send authorized representatives who may claim on their behalf.  Authorized representatives must submit (1) an authorization letter from the applicant, (2) a photocopy of the applicant’s valid ID bearing his/her signature, and (3) the representative’s own valid ID with signature.
2. All unclaimed Transcripts or Records will be disposed after six (6) months from the scheduled date of release.
